Sports Physicals for $10
Perry County Memorial Hospital is offering physicals for $10 on Wednesday, May 29. Please pick up a form, which is located in the pockets on the wall next to the Perryville High School office window. You must call to schedule an appointment. The form will have the phone number, times, and other important information that will need to be filled out prior to having the physical completed, including parent signatures.
